Welcome to the Raiders Fall 2006 Trimester!

No swimmer will be allowed to practice until all forms have been completed and monies paid.

These forms are for all swimmers.

All Swimmers/Parents must complete before swimming:

· Raiders Registration Form/Emergency Contact Form

· CSM Informed Consent

· USA Registration Forms --will be provided at registration (we do not have 2007 forms yet)

Group Evaluations/tryouts are scheduled as follows:

CSM Friday, Aug 25, 4 - 5:45 pm

Monday - Wednesday, Aug 28 - 30, 4 - 5:30

Thursday, Aug 31, by appt only

DAC Monday - Wednesday, Aug 28-30, 6:30 - 8:00

Thursday, Aug 31 and later, by appt only
